IMMEDIATE DENTURES

Any removable dental prosthesis that has been fabricated for placement immediately following the removal of natural tooth/teeth are called as immediate dentures. These are dentures that are constructed before all of the remaining teeth have been removed and are inserted immediately following removal of the remaining teeth. An immediate denture may replace one tooth or all sixteen teeth in either the maxillary or the mandibular arches or in both the arches.

It offers many advantages such as:

  • Reduced edentulous period of healing
  • Reduced pain as the denture acts as a protective splint
  • Less change in muscles and supporting structures and the vertical dimension is
  • restored and maintained properly.
  • Better resiliency and stability of dentures
  • Great esthetics

Types of Immediate dentures

According to the denture flange’s shape

  1. Flanged type
  2. Partial flanged
  3. Open face {socketed type}

According to the surgical alveoloplasty performed

  1. With surgery {surgical alveoloplasty}
  2. Without surgery{surgical alveoloplasty}

According to timing of teeth extraction and denture purpose

  1. Conventional immediate denture
  2. Interim immediate denture
  3. transitional immediate denture

The flanged dentures are retentive and easier to reline and rebase and in open face dentures, we socket the anterior teeth to maintain an acceptable appearance in the post-extraction period by extending the neck of the denture teeth in the sockets. Alveoloplasty is done to remove bony projections that may result in undercuts,improves the path of insertion for the prosthesis and removes bony sources of irritation.

Conventional immediate dentures are a complete or removable partial denture or overdenture fabricated for placement immediately after extraction, the posterior teeth are extracted and allowed to heal before anterior teeth extraction. It is intended as a long term prosthesis and after healing is completed, it is relined. It is generally indicated when only anterior teeth are present or few posterior teeth remain that do not support a partial denture. In an Interim immediate denture, all remaining teeth are extracted on the same day and immediate denture inserted on the same day and after the healing is completed, a second, new complete dentures is to be fabricated as the long-term prosthesis. It is intended as a short term Prosthesis.

Transitional immediate denture are interim immediate dentures that serves as an interim prosthesis to which artificial teeth will be added as all natural teeth are lost. A transitional denture may become an interim complete denture when all of the natural teeth have been

removed from the dental arch. Despite all this these dentures do not fit well as complete dentures which may cause discomfort that may require temporary relining or use of adhesives. The patient is advised to wear the denture for first 24hours without removing it which is further removed by the Dentist.

The steps involved are primary impressions ,fabrication of custom tray, secondary impressions , jaw relation recording ,try in of posterior teeth followed by preparation of the surgical template, extraction and surgery, Immediate dentures are important modality as they provide instant esthetics and function to the patient immediately after extraction. It also provides a psychological advantage at the time of his debilitating loss.
